At anytime if there is any change in your marital status, change of address, phone number, the birth of a child, or a death of a dependent you must notify COBANC within 30 days. If you call our office at 516 794-0600 we will assist you with what is needed to update your information with us. We also have a fillable enrollment form on our website. View it by CLICKING HERE. You can fill out the form print it out sign and date it and send it back to COBANC.
